| Abstract | Abstract : This thesis examines the trends and prevalence of drug-resistant tuberculosis (DR-TB) in the Kingdom of Bahrain between 2013 and 2023, with a particular focus on multidrug-resistant TB (MDR-TB). MDR-TB, defined by resistance to isoniazid and rifampicin, poses a major challenge to global and local public health systems. Adopting a retrospective design, the research analyzed 1,158 TB-positive samples, with 310 identified as drug-resistant cases. Antimicrobial susceptibility testing (AST) was performed using the BACTEC MGIT 960 system on key drugs including Streptomycin, Isoniazid, Rifampicin, Ethambutol, and Pyrazinamide. The findings reveal that MDR-TB prevalence peaked at 5.48% in 2018 and noted significant Pyrazinamide resistance, reaching 32.65% in 2023. The analysis emphasizes a higher incidence among males (69.7%) and individuals aged 21-40 (60.2%), with non-Bahraini, especially South Asian expatriates, being predominantly affected. Regional comparisons imply that Bahrain's resistance rates are in line with those of Gulf Cooperation Council (GCC) countries and stay below global high-burden regions. Furthermore, the COVID-19 pandemic significantly disrupted TB diagnostics, resulting in a lack of resources for AST testing in 2021. These results emphasize the insistent need for enhanced molecular diagnostic methods, effective surveillance systems, and specific public health interventions, along with coordinated regional efforts, to effectively control and reduce the spread of MDR-TB. | |
